1	You must currently be a Medicare Certified, Joint Commission on Accreditation of Healthcare Organizations (JCAHO) Accredited, or Community Health Accreditation Program (CHAP) Accredited Home Health Agency in order to apply as an Ohio Home Care Agency Provider. 2	You must submit the following: Your Medicare Certification Letter, JCAHO Accreditation letter or CHAP Accreditation Letter. Two forms of identification. One form of ID must be a copy of the Social Security Card of the person that will be signing the Application. The other form of ID must be a government issued photo ID of the person that will be signing the Application. The 147 C letter from IRS .	Are credit cards accepted as a form of identification? No.
