You must have a current certificate of completion of a First Aid Training class.

  How do you define current?
Certificate will have an expiration date on it. Therefore, if certificate is not expired it is current.


You must have a current waiver consumer requesting, in writing, your help with personal care assistant's needs.

  Where do I find a Consumer? Most who apply to become Providers already have a friend, family member or neighbor that is a Waiver Consumer. If you do not have a consumer already, you may try the Ohio Home Care website Request for Service tool to locate possible cases. Others have used business cards, advertising, and networking to locate possible Consumers.


You must submit two forms of identification.
1. One must be a copy of your Social Security Card.
2. The other must be a government issued photo ID with your current address on it.

If your ID does not list your current address you may submit a current utility bill or pay stub that has your current address on it along with the copy of your ID.

  Are credit cards accepted as a form of identification? Credit Cards are not acceptable ID and will not be accepted under any circumstances.


You must have obtained within the last 24 months a certificate of completion of either a nurse aide competency evaluation program conducted by the Ohio Department of, Health or the Medicare competency evaluation program for home health aides, or other equivalent program.
