1	If you want to become a Supplemental Transportation provider: Proof of Insurance on the vehicle or vehicles you will be using for Transportation. A current vehicle inspection done by the Ohio Highway Patrol. A picture of the inspection sticker in the vehicle you will be using. You must submit two other forms of identification. One form of ID must be a copy of the Social Security Card of the person that will be signing the Application. The other form of ID must be a government issued photo ID of the person that will be signing the Application. The ID it must have a current address on it. If the ID does not list a current address you may submit a current utility bill or pay stub that indicates the current address on it along with the copy of the government issued photo ID as mentioned. Credit cards will not be accepted as a form of identification.
