What's a Request for Services?

The Ohio Home Care Program posts Requests for Service and Help Needed in order to connect the skills of Providers and the needs of Consumers.

Consumers make the choice of the provider. Then, the Ohio Home Care case manager will work together with all parties to assure that the choices of the consumer are being met.

Supplemental Transportation Providers

Supplemental Transportation Providers are needed in all Ohio Counties to provide transportation for current Ohio Home Care Waiver, Transitions MR/DD Waiver and Transitions Carve-Out Waiver Consumers. Home Modification Services are environmental accessibility adaptations to structural elements of the interior or exterior of a consumer’s home that enable the consumer to function with greater independence in the home and remain in the community.
