Clinical Management Services


Case Managers
assess, authorize services and provide on-going case management of a diverse population of persons with chronic illnesses or disabilities of all types.  RN, LSW or LISW required and have 12 months of home or community based experience within the past 5 years.


Clinical Supervisors
provide case management to Ohio Home Care consumers. As part of the agency management team responsibilities include clinical supervision of direct service professionals; problem-solving with consumers and providers; grievance resolution; working closely with community resources, health care providers and ODM; participation in agency CQI activities; and assuring compliance with regulations and agency policies. RN, LSW or LISW required with a minimum of 36 months home health care and/or community-based experience.


Intake Department
staff receives all inquires and referrals for Ohio Home Care Program services.  The Intake department assures applications are processed accurately, according to OHC Rules and are completed according to ODM contractual requirements.  Also responsible for developing, implementing and overseeing systems to assure all functions of the Intake Department are performed effectively and efficiently.  Monthly and quarterly report preparations, including analysis of intake patterns, compliance with standards and plans for improvement when necessary.  Licensed Social Worker, LISW or RN are preferred.


Program Management Services


The Quality Improvement
staff measures work performance by systematically collecting, analyzing, evaluating and reporting data regarding established quality indicators.  This Includes closely monitoring and tracking progress towards the achievement of goals identified in the Quality Management and Improvement Plan.  The Quality Improvement team assures continuous quality improvement through team development.


Regional Managers and support staff are qualified individuals should have excellent verbal and written communication skills, excellent customer service attitude, demonstrated ability to prioritize and organize task, and detail orientated.  Knowledge of Microsoft Office programs. 


Provider Management Services


Provider Enrollment Managers
have supervisory responsibility for the functions related to the enrollment of ODM providers and assure that all providers meet the eligibility criteria. LSW, LISW, or RN required with a minimum of 24 months experience in home and community based service environment. Experience performing as a manager in a health care or social service setting and excellent communication skills needed. 12-24 months experience with Quality Improvement Systems and Consumer Incident Reporting Systems and knowledge of Medicare, Medicaid, and community resources.


Provider Monitors
are home-based personnel responsible for the annual review of all Medicaid Home Health Care Waiver Independent Providers as well as the investigations of alleged performance violations by Independent Providers. Functions include face-to-face interviews with Independent Providers, review of service documentation and billing, report writing, review and approval of Plans of Correction and record keeping. Qualified individuals are either a RN, LSW or LISW.


Incident Management, Investigation and Response Investigators
are home-based personnel who are responsible for the investigation of protection from harm allegations for consumers of the Medicaid Home Health Care Waivers. Functions include face-to-face investigation and witness interviews, referral and cooperation with other involved agencies or law enforcement, report preparation, plan of correction review and approval and record keeping. Qualified individuals are either a LSW, LISW, or RN. Home and Community Based Services experience (24 months) is preferred.


Home Mod Specialists receive and review requests for accessibility adaptations and adaptive/assistive devices.  Responsibilities include managing this process from request through completion, including determining if the request meets criteria for coverage and overseeing the bid process.  Position also includes education of case managers re: adaptive equipment and home mods, as well as a high level of interaction with providers. Occasional in-state travel required (reliable transportation necessary).  LSW, LISW, OT, PT, or RN with knowledge of disabilities, ADA, DME, Medicare/Medicaid, computer usage and home modifications.
