Important 1099 Tax Form Info!
According to Ohio Health Plan's Provider Network Management, the 1099s for tax information for 2009 were mailed to all providers last week. The United States Postal Service is starting to return the 1099 forms with "bad" addresses. If you moved during the past year and did not receive your 1099, please fax your name, updated address, and provider number to 614-995-5904. All address updates must be signed and dated by the provider.
Also, please remember to file your taxes. All agency and non-agency providers must adhere to Ohio Administrative Code 5101:3-45-10, which states that you are responsible for filing your federal, state, and local taxes. Thank you in advance for your cooperation.
